Questions written by me which should be trivial for humans to answer but hard for bots. In particular, they are unique this forum so bot writers probably won't bother.

Do you think its possible they have a backdoor to your hosting service as a whole?
Could be the issue. If what they wrote above is right.

Nah, I think they are just good at solving captchas. Spammers sometimes also cleverly use actual humans to defeat login challenges. Nothing is 100%.

I changed it to require admin approval for new user creation, in addition to the other measures.

I was thinking the second sentence you said, Arthur, about humans. It's what I thought for every spambot.
Also, doesn't 100% pure gold exist?

There is never pure 100% gold at the macroscopic scale, there are always some impurities.

Also, using humans to make spambot accounts sort of defeats the purpose, so spambot makers usually try to avoid doing that.

As for the admin approval thing, how will you decide which accounts to approve?

Heres an idea:

Set up a server that sends spam mail that has a link that forces you to solve captcha.

Everytime a user accesses the link, an actual site is acess and a bot makes an account
for it, and sends the captcha data to the user.

The user unwittingly solves it, and thus the bot is cleared and an account is made.
